    Previously I have praised the new AccurX system for making requests online. However, recently it has been difficult to put in a medical request because the system seems to get full very quickly. This morning I tried at 8.39am but it was already full. It seems like we are going back to the old times of having to make requests before 8am!

    Dear patient, thanks for your comments. We have this week actually altered the start time for the online to match our core hours which is 8am now and not 7.30 as previously. Some days I admit are easier to contact us than others that depends on patient demand v our clinical safe capacity. However a Monday, as 4th March was, are by far our busiest day every week.     I like the fact that you can now contact the surgery and they get back to you. There should be the opportunity to book appointments in advance as not all appointments are urgent. Ie contraception check with nurse. The system for calling in for a child's appointment is madness. You are normally dealing with a sick child and then trying to call in to be told there are no appointments left.

    Dear patient, thank you for your review, where possible utilise the online form to request 'things' from us so for some issues you can utilise the administration and not medical request form. The new Accurx Patient Triage system is easier for you to submit a request for someone else as you don’t need to sign up for ‘Proxy Access’ you simply complete the form and advise this request is on behalf of someone else, the questions then differ slightly as you tell us about your details and the patient for whom you are completing it, you also get the opportunity to advise whom we need to contact when we are answering the online query; so if you completed a form on an adults behalf it might be that you want us to contact the actual patient. This is the key to the success of the system as it is the most senior and experience clinically trained staff that are reading and reviewing and then ‘signposting’ the requests to the best resource in the system which actually frees up time for our clinical team to deal with the sickest of our patients, while all the other requests for help get dealt with by the right person in the multidisciplinary team. It is true that once we have hit our safe capacity, as we are not an urgent/walk in service you might get signposted to either 111 or the Urgent Treatment Centre for support on occasion.     Despite all the online appointments being taken The receptionist was really helpful and said the GP would call me. He did so within10 minutes and allayed my worries following major surgery

    Dear patient, thank you for taking the time to post us a review, I am glad to read that your appointment was quick and responsive. It is true that we have made changes during last year to our appointment system, the reason for these changes is as a direct result of contractual changes imposed on us by NHS England, some of the changes they have made involve the way we must offer patients an assessment at the time of every appointment request, we have been able to achieve this via the ‘Accurx total triage’ model, where all requests come in the same route to the practice and then each and every patient request is then reviewed and triaged by a senior member of our clinical need, then the most appropriate outcome for each and every request can then be offered, and booked in. The clinical team are working through the (clinical) medical requests, booking in consultations where appropriate for patients or dealing with the patients request in the most clinically appropriate way. This might be making a booking for the patient with another, more appropriate member of our multidisciplinary team, which now includes here at the practice a range of: • Doctors • Advanced Nurse Practitioners • Primary Care Matrons • Nurse Practitioners • Practice Nurses • Health Care Assistants • Clinical prescribing Pharmacists • Care-Coordinators • Social Prescribing Link Workers • Podiatrist • Physiotherapist Every contact starts off with the most appropriate method, taking into account the patients own contact preferences, a face to face appointment will be offered where clinically appropriate and we do offer many of these each day where patients need to be examined for example. It is however true that many of the day to day appointments can be dealt with, without the need for a face to face appointment and this helps us to free up some precious time. The key to the success of the new system as that it is the most senior and experience clinically trained staff that are reading and reviewing and then ‘signposting’ the incoming patient requests to the best resource in the system which actually frees up time for our clinical team to deal with the sickest of our patients, while all the other requests for help get dealt with by the right person in the multidisciplinary team.     I have many medical conditions, under control, so have frequent contact with the surgery. Although it can be difficult getting in touch these days, and booking an appointment is hit and miss, the care I receive is good. Had a blood test 2 days ago, and the nurse who took my blood was, and always has been, awesome. She rarely leaves a mark, and on the rare occasions there is a bruise, it's because my veins are hiding from her. I always breathe a sigh of relief when I see her name appear.
