Personal health records

Information:

We have changed how we talk about accessing certain NHS services online. We now call this logging in to your NHS account, whether you use the NHS App or the NHS website to do this. The services we offer have not changed.

A personal health record stores information about your health and care online. It's managed by yourself, and health professionals can add information to it.

Your GP surgery or hospital contracts with one or more providers to deliver personal health record services. The providers are commercial organisations. They provide a service on behalf of your surgery or hospital.

We currently support personal health record services with Patients Know Best Ltd and Substrakt Health.

Access personal health record services in your NHS account

If your GP surgery or hospital offers personal health record services, you may see any of the following options:

  • consultations, events and messages
  • hospital and other appointments (view appointments)
  • shared links
  • care plans
  • hospital and other medicines
  • track your health
  • record sharing
  • update your personal details
  • ask your GP surgery a question
  • patient participation group

Patients Know Best Ltd

The following personal health record services are available from Patients Know Best Ltd.

Consultations, events and messages

See details of your visits and treatments, view clinical documents, message your health team, or fill in a consultation form.

Hospital and other appointments

View your upcoming and past hospital and healthcare appointments. You can also add details of other appointments to your record.

Shared links

View useful links your doctor or health professional has shared with you. For example, your doctor may give you links to information that helps you manage a condition like diabetes.

Care plans

View and manage your care plans. A care plan helps you manage a condition and your health. It usually includes an action plan, with information about your care, health goals and advice on monitoring your conditions.

Hospital and other medicines

View your current and past medicines or add a record of your own.

Track your health

Track your health lets you monitor your health and symptoms online and record how you’re feeling.

Record sharing

Choose and manage information you share with your health teams.
