A personal health record stores information about your health and care online. It's managed by yourself, and health professionals can add information to it.
Access personal health record services in the NHS App
Personal health record services are a new feature in the NHS App. We're testing it with a small number of GP surgeries and hospitals. We will go live with more in 2020.
If your GP surgery or hospital offers personal health record services in the NHS App, you'll see the following options:
- messages and consultations with a doctor or health professional
- hospital and other appointments (view appointments)
- shared links
- care plans
Messages and consultations with a doctor or health professional
Messaging lets you contact a doctor or health professional online. You can send them a non-urgent query and get a response. It saves you having to phone or wait for an appointment.
A consultation is a questionnaire or online form you fill in about you and your health. It’s sent to your doctor or health professional to review. For example, your doctor may ask you to complete a pre-appointment form before you go to an appointment.
Hospital and other appointments
View your upcoming and past hospital and healthcare appointments. You can also add details of other appointments to your record.
View useful links your doctor or health professional has shared with you. For example, your doctor may give you links to information that helps you manage a condition like diabetes.
View and manage your care plans. A care plan helps you manage a condition and your health. It usually includes an action plan, with information about your care, health goals and advice on monitoring your conditions.
Personal health record service providers
Your GP surgery or hospital contracts with one or more providers to deliver personal health record services. The providers are commercial organisations. They provide a service on behalf of your GP surgery or hospital.

We only support queries about Patients Know Best relating to their services in the NHS App.
