From the home screen:
- Select Profile.
- Select Care plans.
This is not available to all users and depends on your GP surgery.
If you use family and carer access in the NHS App, you cannot view care plans for that person in the app.

You may be able to view your care plans in the NHS App. Care plans show how you want to be cared for and supported.
