We've updated the NHS App to make it easier to use. If you're still using the old version of the app, you'll find documents by selecting GP health record on the home screen.
Find out what's new in the NHS App
GP documents
To view documents from your GP surgery:
- Select Documents.
- Select Your documents.
- Read through the important notice about sensitive information you may see and select continue.
You can see documents added by your GP surgery, such as:
- referral letters
- discharge documents
- reports from scans
- Images you’ve shared with your GP surgery
- letters from your hospital sent to your GP surgery
Hospital and specialist documents
To view documents from your hospital or specialist:
- Select Documents.
- Select Hospital and specialist documents and questionnaires.
You may also be able to access your documents and questionnaires from your Appointments screen.
You can see documents added your hospital or specialist, such as:
- pre-appointment questionaries
- appointment letters
- outcome letters from appointments
Questionnaires in the NHS App will:
- take 3 to 5 minutes to complete
- save, so you can pause and come back later
You can change an answer before you submit a questionnaire. if you need to change anything after you have submitted your questionnaire, contact your healthcare provider. To finish an incomplete questionnaire:
- Go to Documents.
- Select Manage hospital and specialist documents and questionnaires.
- Select the In progress questionnaire you want to complete.
Previewing and downloading your documents
Some documents cannot be previewed within the NHS App and will need to be downloaded to your device to view.
For Apple devices, Apple has a guide on viewing and managing files on your device.
For Android devices, Android has a guide on finding and opening files on your device.
When you download a document, it becomes your responsibility to keep it private. If you share your device with someone else, or if you’re using a public computer (like in a library), other people might be able to see your health information.
To protect your privacy, only download medical documents onto a device that only you can access. If you’re using a shared device, make sure to delete any downloaded files before you log out of the device.
Unavailable documents
Some documents may show as being unavailable in the NHS App. Contact your GP surgery to request these documents.
If you cannot find a document or questionnaire from your hospital
Some healthcare providers do not share hospital documents and and questionnaires in the app.
If you cannot find a document or questionnaire you expect to see in the app, check your messages as it may be there.
Contact your healthcare provider if you’re still having problems finding it after looking there.
Incorrect or missing information
If any information in your documents is incorrect or missing, contact your GP surgery.
