   Notes written on app about conversations/appointments incorrect. Details sent in letter incorrect. Prescription went through at too low a dose. Almost impossible to get the problem fixed. Difficult to get through on the phone. Argumentative and rude receptionists.

   No communication between prescription secretary & doctors. Failure to inform patient of changes made by the surgery to their prescription. Only find out when the prescription is not full filled correctly.
