    My mother died in a nursing home on 22/02/25. She was attended by doctors from this clinic who were, it seems, rather reluctant to ascribe a cause of death. After intervention by Southwark coroners, a cause of death was finally given as frailty in old age and heart failure. Southwark coroners instructed this practice to forward the paperwork on 28/02/25. Granted, the weekend was a factor, but by today 05/03/25 it still hadn't been done - and would still not have been done had I not attended the medical examiners office at Lewisham Hospital and found out that this practice still hadn't forwarded the paperwork. I called the practice and found out that the paperwork was together, but had not been sent despite 1 mail from the coroner and 2 from the medical examiners office. Why was it even necessary for me to intervene to get things done? Have you any idea how upsetting this is? Why put bereaved relatives through the stress of lackadaisical record keeping and sitting on paperwork for no reason? Please explain. The receptionist said that she would sent the paperwork through today. How long would I have been waiting had I not pushed things along? It's only forwarding paperwork after all.

We are deeply sorry for the distress this has caused you and sincerest condolences on the loss of your mother. Since September 2024, an entirely new step has been introduced into the process of certifying deaths, known as the Medical Examiner’s Office. This is a new referral process, requiring doctors to refer a case to the medical examiner before being allowed to issue a Medical Certificate of Cause of Death (MCCD). Once the medical examiner completes their review and confirms there are no concerns, they instruct the GP to issue the MCCD. Under these regulations, only a GP who has previously treated the patient during their lifetime is authorised to issue the MCCD. Unfortunately, in this case, the GP who had treated your mother was on annual leave at the time of her passing. We would like to invite you to meet with us in person so we can explain our investigation in detail. However, if you prefer not to attend in person, we can provide our findings via email.
