   It would help me to register with this practice if the registration form for new patients was available on line.

   Our website www.martinsoak.co.uk has a quick link on front page called New Patients - if you click on this link it takes you to an online registration form and new patient health questionnaire. Almost 90% of new registrations at the surgery are done online now although we do have printed copies at the reception desk for those not able to access via a computer or smartphone.
