    I really am impressed with this practice, reception staff are helpful and efficient with requests, it has been easy to register and get appointments. I was seen on time and by a very helpful doctor, where I was referred quickly to other services. Really impressed.

  2. Review titled End of life care at home for my husband

    My husband had Parkinson's disease. He was diagnosed in 2011, gave up work in 2012. I took on more care of him from 2017 onwards. He was admitted to Musgrove Park Hospital because Covid caused a steep decline and was on four different wards before the end of the month. This was mostly due to pandemic pressures. Triscombe Ward where he ended up was brilliant. They enabled him to leave hospital for a couple of hours to attend his daughter's wedding. He lost a lot of weight and a peg tube was fitted so he could receive adequate nourishment despite ongoing fatigue. He contracted pneumonia, which the ward treated very promptly with oxygen and intravenous antibiotics so that he improved rapidly. It was clear that any care he would receive was palliative, since he did not respond to rehabilitation treatment. Although communication with him was difficult, he made it clear he wanted to be at home. I felt I was involved with all the decisions about his long term care. I found the transition from hospital to home made me a bit anxious because all questions were referred to Discharge 2 Assess Team (D2A). It would have helped me to have been able to have had contact with a representative of that team before he left hospital, which would have allayed certain fears. I became responsible for administration of my husband's nutrition and medication, while it could be passed through the PEG tube but all other personal care was supplied by the carers. I was provided with the equipment, supplies and training I needed. Carers were provided four times daily. They were a lovely bunch of people and quickly became trusted and valued almost as members of the family. The District Nurse team added my husband to their list and provided me with 24 hour contact details. I have nothing but praise for the District Nurse team. They supported me very well - finally at least daily when he was put on a syringe driver to deliver pain management and other medication. Whatever time of day or night I called them, they always seemed happy to come and encouraged me to get in touch if I was concerned about anything. My husband contracted pneumonia at home. Our local GP surgery was very supportive and always responded to any application for help. My husband had three different antibiotics but, although these provided some alleviation of symptoms, the infection persisted. My GP, I think, applied for CHC funding, which paid for the cost of care (through Butterfield) and also provided some night sitting towards the end. This was a great service as I could sleep without listening for changes in his breathing. All the carers we had contact with were dedicated individuals who provided an excellent service. After battling with the infection six weeks, my husband passed away. He was able to stay at home to the very end, which would not have been possible without the support of the District Nurse service, carers and my GP.
