Service type

NHS Health Checks are available to eligible West Berkshire residents at their GP Practice. Some residents will be eligible for an NHS Health Check through the Community Wellness Outreach Service. If you are eligible for this outreach service your GP Practice will contact you.

Referral method

If you are 40-74 years old without a pre-existing condition and are registered with a GP you should receive a letter and/or text inviting you for a free NHS Health Check every 5 years. Once you get your invitation follow the instructions to book your appointment. You can also ask your GP Practice to see if you are eligible for an NHS Health Check provided by your GP Practice or the Community Wellness Outreach Service.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
