You are only entitled to an NHS Health Check from the local authority in which you live.
This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

If you are aged 40 to 74 and you don't have a pre-existing condition (such as diabetes) you should be invited by text (letters are available too) to an NHS Health Check at your GP practice once every 5 years. Once you receive your invitation please follow the instructions to book your appointment. You can also ask your GP practice to see if you are eligible.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

The NHS Health Check service is available at GP practices in Southwark. NHS Health Checks can also be received in our hub in the north of the borough.
