Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

NHS Cardiovascular Disease (CVD) Health Checks – Outreach

A free NHS Health Check is a quick and simple way to check your overall health and help you take action to reduce your risk of developing serious conditions such as heart disease stroke diabetes kidney disease and dementia.

Our practitioners will measure your blood pressure take your height and weight measurements check your cholesterol and blood glucose levels. They will also discuss your lifestyle. From these checks you will be given a risk score and that indicates the likelihood of having a cardiovascular event in the next 10 years. They will help you understand your results and what they mean and provide advice and support on staying healthy. The practitioner will also refer or signpost you to other services if needed.

The whole appointment takes 30 minutes so if you are aged 40-74 a resident in Somerset AND / OR registered with a Somerset GP have not had an NHS Health Check already in the past 5 years and have no pre-existing conditions or on certain types of medication you are eligible for a health check!

The health checks are currently taking place across Somerset in Wells Yeovil Chard Glastonbury Taunton Langport libraries and at the SHAPE Mendip Council Offices. Details of locations and booking links can be found here: Health Check locations . There will be other locations added in the future.
