This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

All Sandwell GP practices offer NHS Health Checks. This service is by invitation letter from the practice or you can ask the surgery directly for an appointment. Community based NHS Health Checks are coordinated by Mytime Active who can be contacted on 0121 366 0966 or via email at sandwell.lifestyle@nhs.net.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

The NHS Health Check service is available at GP practices and in the community across Sandwell Council catchment area.
