To book an NHS Health Check you will need to contact your GP surgery directly. They can advise on your eligibility and provide details on appointment availability.

Referral method

If you 40-74 years old without a pre-existing condition and are registered with a GP you will automatically be invited once every five years. Once you get your invitation the patient must follow the instructions given to book their appointment. Alternatively the patient can request to have their NHS Health Check by contacting their GP surgery.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
