You are only entitled to an NHS Health Check from the local authority in which you live.This service is available to eligible residents or patients registered to a GP in this catchment area.
Referral method
Patients will be invited for an NHS Health Check by text/letter initially and within this invite they are asked to reach out to their GP to book an appointment. Bexley Health Neighbourhood Care team (GP Federation) will then follow up these invites by phoning patients already invited to book them in to clinics when needed.

Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
Service type
NHS Health Checks are offered primarily within the GP surgery either within normal clinic hours or within the extended access clinics offered (evenings and weekends). Further community clinics will be available where need arises to support the wider network and achieve the target.
