Description
This is a home visiting service. Our Co-ordinator will talk to everybody in the household to help them understand the physical and psychological effects of a stroke. The Co-ordinator will also give practical advice on caring for the person at home. The service also recruits and supports Peer Support Volunteers and an exercise and education programme.
