Eligible Lancashire residents can access NHS Health Checks at their GP or via FCMS (NW) our community outreach provider https://nhshealthcheckslancashire.co.uk/

Referral method

Eligible residents will be sent an invitation via text or letter by their GP. Residents can also contact their GP to receive an NHS Health Check or they can click on the following link to book an appointment with the community outreach team https://nhshealthcheckslancashire.co.uk/book-now/

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
