You are only entitled to an NHS Health Check from the local authority in which you live.This service is available to eligible residents or patients registered to a GP in this catchment area.
Referral method
Some of our community providers actively work with GPs and PCNs so that general practice send out a targated text invite / info on where they can get an NHSHC. This does not happen with all PCNs across the ER. Individuals can also contact the providers directly or the ERYC Public Health Team and information will be provided. All community providers actively promote NHSHCs and where they will be delivered within the East Riding of Yorkshire. All providers also offer opportunistic NHSHCs in conjunction with the services they provide across the East Riding. As you can also see from the websites (previous answer ) individuals can contact providers directly to book an NHSHC.

Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
Service type
The NHSHC is available through a number of Community Providers that include our commissioned Integrated Lifestyle Service (Xyla) East Riding of Yorkshire Leisure Services & Humber NHS Teaching Foundation Trust (YourHealth). All of the above deliver NHSHCs at a variety of locations throughout the East Riding of Yorkshire. However we provide NHSHCs via an Any Qualified Provider route so technically anybody who meets the criteria that we have in place to deliver can do so subject to ERYC approval. As such we also have a number of other providers including a small amount of GP Practices (Pocklington Group Practice) a couple of Community Pharmacies (Westhill Pharmacy Bridlington & Marton Road Bridlington) & several other community providers currently delivering NHSHCs within the East Riding of Yorkshire (East Riding Physio) (and currently in the process of approving Yorkshire Health Partners) https://yorkshirehealthpartners.co.uk/
