You are only entitled to an NHS Health Check from the local authority in which you live.This service is available to eligible residents or patients registered to a GP in this catchment area.
Referral method
The FREE NHS Health Check is available to adults aged between 40 and 74 (without pre-existing conditions) once every five years. It accesses your overall health and can help reduce your risk of developing heart disease diabetes stroke kidney disease some cancers and dementia.
If eligible you will receive an invitation from your GP practice in due course.
If you think your check may be overdue please contact your GP practice.

Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
