This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

Patients are invited by their GP practice to receive an NHS Health Check. Providers are expected to be proactive and creative in promoting attendance. National template letters posters and information leaflets may be used/ adapted for the initial invitation subject to the provisos for public facing material below and by agreement with Bury Council. Recall may be via a letter phone call or text message.

Onwards referral to lifestyle services should be read-coded.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

NHS Health Checks is strictly GP led some pharmacies measure blood pressure BMI but do not conduct full NHS Health checks.
