Welcome to Meltham Village Surgery.

On April 1st 2016 Elmwood Family Doctors merged with Meltham Village Surgery.

Backed and supported by the Greater Huddersfield Care Commissioning Group and NHS England this merger will enable services to be maintained at the practice in Meltham. Dr Mike Pacynko successfully ran Meltham Village Surgery  for over 20 years. He retired on 31st March 2017.

The merger also offers an exciting future for the patients of both surgeries. We will continue to work with the Patient Participation Groups in Meltham and Holmfirth to maximise the potential of the larger team of professionals and sites. Initially the priority is to ensure that the existing patients of Meltham Village Surgery have the same and whenever possible even greater access to their team of health care professionals. As things progress we envisage being able to offer the patients in both villages more variety and choice of how and when they access the increasing number of services we will be able to provide.

The two sites of the practice will be referred to as “Elmwood Family Doctors – Holmfirth” and “Elmwood Family Doctors – Meltham”. Patients will continue to use their own surgeries as normal and contact the surgeries on their existing numbers. Please keep an eye on this site and our social media pages for future updates.

Appointments may be available before 6.30pm in some areas.

To book an appointment, talk to your GP practice or visit their website. You may be able to get an appointment on the same day if required.

Depending on your area, you may be offered an appointment at one of these locations:

On a Wednesday we are open to 8.00pm for pre-booked appointments only.

If you need to contact the NHS for urgent care there are THREE numbers you need to know; 999 for life threatening emergencies, 111 for fast medical help which is not an emergency and your GP surgery.  Please remember the hospital A&E Department is for Accidents and Emergencies ONLY.

When the surgery is closed an emergency service is available.  Please telephone 111.  You may be offered advice over the phone, offered an emergency appointment at the Local Care Centre or a home visit may be arranged.

This service is provided between 6.30 pm and 8.00 am Monday to Friday, weekends and Bank Holidays.

Access to Patient Online services

The practice provides patient online access to practice information by using electronic communication for the booking and cancelling of certain appointments with a doctor and also ordering of repeat presciptions.

Should you wish to register for this service, please  bring some photographic identity to ths surger as recommended by the Royal College of General Practitioners.  You will then be issued with documentation on how to receive your personal password to access the system.
