If the telephone lines are busy and you are having difficulty getting through to the surgery via our phone lines in the hours specified, please feel free to register for online appointments.

Please download and print an online appointment form from our our new patient page, you will receive a username and password, within 72 hours. 

You will also find other useful information about the practice from the Documents Library page on the website also.

If you have any medical queries, or are unable to get an appointment, you are also able to use FEDNET Out Of Hours Services - please see the website and the signage on our practice walls, as well as our practice leaflet for guidance.

People living in Waltham Forest are  able to make an appointment to see a GP in  evening at weekends.

WEEKDAY EVENINGS

GP evening appointments are available Monday to Friday from 6.30PM to 10PM.

Evening appointments will take place in Leytonstone.

WEEKEND

GP weekend appointments are available Saturday and Sunday from 8AM to 8PM.

GP appointments are available at;

  • Chingford: 10am-4pm (Saturday), closed on Sundays
  • Leytonstone: 10am-4pm (Saturday), closed on Sundays  
  • Walthamstow: 8am-8pm (Saturday and Sunday)

If you have been referred to a hopsital or clinic, you are able to book the appointment online. You can book your own appointment, on a date and at a time that is conveniant for you, you can also choose the hopsital or clinic that you want to go to. Once you have booked your appointment, you can check your details, change your appointment or cancel it if, it is no longer needed.

The service is fast conveniant and secure Booking your own refertal appointment online helps save your us time and also saves the NHS money. 

When patients book their own appointment, hospitals have said that there are half as many missed appointments, booking online also saves the NHS on phone bills and sending letter out to you. 

We know that the service is not for everyone, therefore you will be able to ask your GP if you need any help. 

MANAGING YOUR REFERRAL 

If you are referred to a hospital or a clinic, you can book your appointment online, from;

  • Your booking reference number, which can be found at the top of booking letter 
  • The password you received at the time your received your booking letter 
  • Your year of birth

If you have been referred, and have forgotten you booking reference or password, please contact the practice. 

If you have any problems booking online, you can call the NHS appointment line on 034 5608 8888. 

Phone lines are open; 

  • Monday to Friday - 8.00AM - 8.00PM
  •  Saturday and Sunday and on bank holidays - 8.00AM - 4.00PM

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

EVIDENCE THAT YOU ARE SICK

  • If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
  • Your employer can ask you to confirm that you've been ill.You can do this by filling in a form yourself when you return to work. This is called self-certification.
  • If you're sick and off work for more than seven days, your employer will probably ask for proof of your illness. Most employers ask for a fit note from your GP.
  • However, this will also depend on your employer's company policy on sick leave (or sickness absence). This policy should tell you how many days you can be off sick before you need to provide proof of illness or a fit note.
  • You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

SERVICES

GP online services allow you to access a range of services via your computer or mobile. Once you have signed up, you will be able to;

  • Book or cancel appointments online with a GP or nurse
  • Renew or order repeat prescriptions online
  • View parts of your GP health record, including information about medication, allergies, vaccinations, previous illnesses and test results

The service is free. Everyone who is registered with a GP can have access to their practice's online services.

HOW CAN I START USING GP ONLINE SERVICES
Follow the steps below to sign up for GP online services:

  1. Tell your GP practice that you would like to start using their online services
  2. A member of the practice will then ask you to fill in a short registration form
  3. You will have to provide photo ID and proof of address. If you do not have any ID then either a member of staff
  4. will have to confirm your identity or you may have to answer questions about personal information in your GP record 

Once you have signed up, you will receive a letter with your unique username and password and a link to where you can log in.

New Patient Registration

NHS

Once all registration forms have been completed you will be given a health check appointmentn with our health care assistant. This is the opportunity to obtain important medical history until your notes arrive from your previous GP.

The health check must be booked a month in advance of your application. Everyone 5 and over should have the new patient health check.

New patients who fulfil government criteria of eligibility to NHS care are able to register with this practice, after an application process, with this practice.

  • All new patients over the age of 5 years will be offered a health check appointment with the practice nurse
  • Patients over the age of 75 are seen annually in the surgery or visited at home
  • All patients over the age of 75 now have a named GP at Dr Shantir’s Practice. Patients can enquire about this at our reception

You will also be asked to provide;

  • Your medical card (from GMS1 to be completed if this is not available)
  • Photographic proof of identification - driving license, passport
  • Proof of address (dated within 3 months)
  • If registering children under 10 years of age, please provide copies of all immunisations

The National Health Service provides health care in the UK and is funded by taxation.  Asylum seekers are entitled to access NHS care without charge while their claim or appeals are being considered.  You need an HC2 form, provided by the National Asylum Support Service (NASS), or available at pharmacies, to receive free medicines, dental treatment, eye tests and some glasses.    

None of the people who work for the NHS, including doctors, nurses and interpreters, will pass on any information about you to any other person or organisation without your permission.  All medical care is confidential and does not affect the judgement on your claim for asylum.
