Patient Access to viewing GP Medical Records

As of 1st October 2014, patients will be able to view on-line, export or print any summary information from their records relating to medication, allergies and adverse reactions. 

Patients will need to register for Patient Access in order to have access to the above mentioned.

Flu Clinic will start on 21/09/2017.  If you are 'at -risk' patient, you can book your flu vaccination now.

All children aged 2,3 and 4 years old 31 August 2017 is eligible for a free intra-nasal flu vaccination. Please contact the surgery at the end of September to book  appointment.


Meningococcal (MENC) freshers Vaccination

Prospective students entering University for the first time  are at increased risk of contacting Men C disease.

Those who never received Men C vaccination or were previously last vaccinated under the age of ten years are eligible.

patients who are aged 16  between 1 April 2014 and 31st October 2014  can be vaccinated during that period provided they turn 17 by 31st March 2015.


Herpes Zoster(Shingles) Vaccination

Aged 70,78 or 79 on 1st September 2014

If you were born on or between;  02/09/43  to 01/09/44,  02 /09/35  to 01/09/36  or 02/09/34  to 01/09/35.

please ring the surgery  if you are eligible for a free shingles vaccination.

Improving access to general Practice:

All patients registered with a GP Practice in Southend can now book appointments to see a GP or Nurse at the weekends(on Saturday and Sunday) at Southend Medical Centre(01702 333298) and at North Shoeburyness Surgery(01702 297976). The service offers access to a GP between 10 am to 3pn and Nurse between 10 am to 2pm.

Appointments need to be prebooked via the practice, but they can be booked on the same day.

This service is being delivered by a local GP Federation. GP Healthcare Allaince.

Patient Information re CARE.DATA

Under the Health and Social Care Act 2012, NHS England has the power to direct the HSCIC to collect information from all providers of NHS care, including general practices.

GPs are legally required, under the Health and Social Care Act 2012, to provide data to the Health and Social Care Information Centre (HSCIC) for the care.data programme.

Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.

This information along with your NHS number, postcode, gender and date of birth but not your name, are sent to a secure system where it can be linked with other health information.

This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.

You have a choice.

You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances. If you do not want information that identifies you to be shared outside your GP practice, please notify us in writing so that we could make a note of this in your medical record. This will prevent your confidential information being used other than where necessary by law, (for example, if there is a public health emergency).

You will also be able to restrict the use of information (Summary Care Records) held by other places you receive care, such as hospitals and community services. You should let us know if you want to restrict the use of this information. Your choice will not affect the care you receive.

Do I need to do anything?

If you are happy for your information to be shared you do not need to do anything and you can change your mind at any time.

If you are not happy for your information to be shared, please inform the Practice in writing. You can download an opt-out form by clicking here. Please complete this form and hand it in to reception.

It is important to note that if a patient has opted out of having a Summary Care Records (SCR) then this preference will NOT automatically apply to care. data extractions (see Summary Care Records on page 25).
Consent sought for the SCR was for that specific purpose only.  If a patient wishes both to opt out of the SCR and to prevent confidential data from being used for wider purposes beyond their care, then they MUST inform the Practice so that it could apply both the SCR dissent code and the care.data objection code(s) in the patient’s medical record.
