You are only entitled to an NHS Health Check from the local authority in which you live.
This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

If you are 40-74 years old without a pre-existing condition and are registered with a participating GP you should receive an invitation for a free NHS Health Check every 5 years. If you receive an invitation you should follow the instructions to book your appointment. If you have not received an invitation but believe you are eligible you can also contact your GP surgery directly or one of our community providers such as West Sussex Wellbeing.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

The NHS Health Check service is available at many GP practices selected pharmacies and in the community across the West Sussex catchment area.
