The Office of the Public Guardian (OPG) helps people in England and Wales to stay in control of decisions about their health and finance and make important decisions for others who cannot decide for themselves.

To make a lasting power of attorney (LPA), you will need to complete an official form ( https://www.lastingpowerofattorney.service.gov.uk/home) and OPG also has a guide (https://www.gov.uk/government/publications/make-a-lasting-power-of-attorney/lp12-make-and-register-your-lasting-power-of-attorney-a-guide-web-version) to help you fill out and register your LPA application
