The NHS Health Check is available at all GP Practices across the Local Authority catchment area
Referral method
If you are 40-74 years old without a pre-existing condition and are registered with a GP residents should receive a [letter/text] inviting them for a free NHS Health Check every 5 years. Once they get the invitation they can follow the instructions to book appointment. They can also ask their GP surgery to see if they are eligible.
Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
