Contact your GP to book an NHS Health Check

You are only entitled to an NHS Health Check from the local authority in which you live.This service is available to eligible residents or patients registered to a GP in this catchment area.
Referral method
If you are 40-74 years old without a pre-existing condition and are registered with a GP you should receive a letter text or phone call from your GP practice inviting you for a free NHS Health Check once every 5 years. Once you get your invitation follow the instructions your GP gave you to book an appointment. You can also ask your GP surgery to see if you are eligible.

Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
