This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

1. Targeted delivered by GP Federation - a) pre-identification of eligible individuals registered (in EMIS) across Haringey GPs with high risk factors for CVD including current smokers people with recorded depression and anxiety people aged over 55 people who are overweight or obese etc. b) send batch messages c) make follow ups using phone d) individuals receive links so that they can self-book using EMIS platform e) they are give available slots and locations/venue to choose from for booking

2. Health check delivered by GPs - GPs send out to invitation to eligible individuals in their GP register and more or less adopt universal approach and appointments are usually in their own surgery.

3) Opportunistic health checks at events and community centres/libraries using POCT or they given links to self-book their appointment.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
