You are only entitled to an NHS Health Check from the local authority in which you live.
This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

If you are aged 40-74 without a pre-existing cardiovascular condition and you are registered with any of the GP practices in Calderdale you may receive a letter or text message inviting you to book an appointment for an NHS Health Check at your GP practice. This is usually in the month after your birthday after you turn 404550 etc.

Not everyone that is eligible for an NHS Health Check will be sent an invitation. If you think you are eligible for an NHS Health Check and you have not already had one within the previous 5 years you can contact your GP practice to check if you are eligible and book an appointment.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

NHS Health Checks are available at all of the GP practices in Calderdale. Occasionally there are pop up health check events at other community venues across the Borough.
