The NHS Health Check Service is available at GP practices. A patient who is eligible for a NHS Health Check can contact their registered GP Practice to arrange/book a NHS Health Check
Referral method
If you are 40-74 years old without a pre-existing condition and are registered with a GP you should receive a [letter/text] inviting you for a free NHS Health Check every 5 years. Description of service
The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.
