    Two people from the therapy team turned up yesterday and sorted out many things for me. They were very professional. They were at my house for quite a long time. They did lots of things on the phones.

    I am so pleased to hear that the therapy team were able to sort out many things for you and you found them to be professional.   2. Review titled Insulin administration

    I'm very happy with the service. The help and support that Winchester rural south give me is very important as I may find it difficult to remember what insulin to take which would have an impact on my health. I enjoy seeing all the girls every day as they are the only people I see.

    I was discharged from hospital following kidney failure and facial shingles. Two hospitals were involved, Winchester and Southampton General.. Neither hospital arranged a care plan. My GP arranged care for me from Frailty. I was confused and worried. I did not feel right. I was almost immobile for a week. Your Frailty nurses and a doctor visited me. They have been a Godsend. Lovely people who have given me care and confidence.

    It took a little while after leaving hospital for the community Physio and OT team to start visiting at our home, but when they did the quality of therapy was outstanding! They initially visited once a week, either the OT or the Physio, but then increased to twice a week after the summer holidays. The team were very knowledgeable, encouraging and supportive. They gave us confidence,encouraging us to strive for improvement and it worked brilliantly well. An amazing group of highly talented, professionals who really care. The only negative is that there are too few of these remarkable people which means the scope for continued improvement is frustratingly time limited and many will not achieve their potential recovery as a result. The quality of the therapy team is excellent but the lack of resourcing/funding is depressing for those who really want to recover to their full potential.

    My mother's GP at the North Baddesley Surgery arranged for the Acute Frailty Team to visit my mother at her home. They attended within quite a short period following the conversation with the GP. They examined her to understand her current condition as well as taking a blood sample and undertaking some tests on my mother and getting a more detailed understanding of the timeline and her current symptoms, explained the situation in more detail. I was concerned about my mothers needs and support in the immediate future and indeed on questioning my mother uncovered the scope of support needed for her to continue live at home in the short term. Overall I felt very satisfied with the the team and the decision to help her stay at home rather than attend hospital. I felt very satisfied with the establishment of a DNR for her, which is now kept at her home with her. The teams suggestion that Hampshire County Council's support service would be requested by the team to provide temporary support was also agreed.

    I was referred for for persistent knee pain and associated loss of mobility. The healthcare professional was well acquainted with my joint history and ongoing concerns. I was given ample opportunity to discuss everything, including recent deterioration. The student healthcare professional was also very involved and listened intently to the summary of the problems. It was made very clear to me that the decision to progress to the next stage to discuss knee replacement surgery was entirely down to me. I did not feel at all pressured, but well- informed and well understood. A very encouraging and informative consultation, thank you.

    My dad has had several falls and I had contacted the surgery to ask the service to come and see us. They were very professional and kind and caring. They took time to listen and we felt listened to. They offered advice and reassurance and said to feel free to contact them. They also set up contact with occupational health. Excellent service we feel so much better.

    I’d like to share my feedback about the Frailty team in Lymington in Hampshire, they have been coming to see me and have now stopped. I was told they would visit me by a doctor and there is a doctor amongst them, and most of them are well experienced doctors or nurses, they came to see me everyday in my home and they basically tell you what you should be doing or shouldn’t be doing. They came to visit me in my home every day and took my vital statistics and all the rest of it, and basically told me to behave, which is difficult for me! At Lymington hospital I had various examinations and they found out that I have quite a lot wrong with me and I needed to take various tablets and do various things and the frailty team come to check that I was doing what I should be doing and that I wasn’t getting worse. When you are unwell, and feeling pain in unusual places, or disturbed by hiccups night and day, along with various other pains then; consultation, explanation and care and attention are so important, these are just some of the things which greatly improve your wellbeing. Yes, this is what you get from the frailty team, it is accompanied by care which is hard to describe but so honest, you know that you are getting not only thoughtful help with no beating around the bush but knowledgeable, down to earth information and treatment. They are also a joy to be with and bring with them warmth and sympathy that is so natural and all so down to earth. I actually looked forward to their visits because they brought with them everything that was meaningful and missing. My wife and I are in a better situation because of their care and attention and straight forward way of working, especially with awkward so and so's like me, thank you boys and girls, I am a different person because of you and your ways.
