Leave a rating and review online
You can leave a rating and review of any NHS service on their profile page on the NHS website.
To do this:
- find the service you want to review
- open the service profile
- go to the 'leave a review' page
- use the online form to leave a rating and review of the service

Give feedback in person
You can give feedback by speaking to a member of staff at the NHS hospital, GP surgery, dentist, opticians or pharmacy you went to.
