Muirhead Dental Practice has always made every effort to ensure patients feel calm and relaxed during their dental visit.

All surgeries are by appointment only. If you cannot keep your appointment, or no longer require it please try let us know as soon as possible as someone else may need it. Muirhead Dental Practice is currently accepting new NHS patients. Please call into the practice to complete an application form or contact reception to register on 0151 256 9404 or 0151 256 6497. We have good availabilty in the next 2 / 3 weeks for an appointment.

Charges for students and those in full time education

If you are 19 years of age (or older) and are in full time education (college or university) and DO NOT have a valid HC2 certificate or are in receipt of any other benefits or named on a valid exemption certificate; you will need to pay for your dental treatment.

You are entitled to free NHS dental treatment if on the first day of treatment you are:

  • Under 18 years of age
  • You are 18 years of age AND in full time education

Information for New Patients

  • Our NHS opening times are between 9am and 5pm, Monday to Friday.
  • It is our practice policy that you must register in person if you are over 16 years of age; you must return the registration form back in person.  The practice, at its discretion will make exceptions on a case-by-case basis.
  • You are NOT registered at the practice until the dentist has seen you.
  • If you have registered and have an appointment booked, but have toothache please contact us on 0151 256 9504 / 256 6497 for any cancellations. However, priority may be given to our existing patients and you may be directed to NHS Englands emergency number on 0161 476 9651. You need to advise them you are not registered with a dentist. You should STILL KEEP your first appointment at the practice if you have made an appointment.
  • Please complete the Medical History Form you’ve been provided and ensure you bring this, and a copy of any medication you may be currently on to your first appointment.
  • If you have to cancel an appointment, please PROVIDE US WITH AT LEAST 24 HOURS NOTICE in order for it to be offered to someone else.
  • If you pay for your treatment, it is our practice policy that you must pay any NHS charges BEFORE you see the dentist on the day of your appointment.
  • If you have any special requirements, for example you require a downstairs surgery or translator services, please let us know in advance of your appointment so these can be arranged.
  • If you believe you are EXEMPT from dental charges, please check this before you attend your appointment. A full list of exemptions is available on reception. You should bring with you on your first appointment your proof of entitlement.
  • You may be able to apply for reduced charges by completing a HC1 form (please ask reception for one). Please note this can take a few weeks to process and to be returned to you. If this is not returned to you before your appointment, you should pay for your treatment and then claim this back. We will provide you with the necessary receipts.
  • Please make sure you read and understand your Personal Treatment Plan. If you are unsure, please ask the dentist. 
  • If you commence dental treatment and you do not return within a 60-day period, you may need to pay for your treatment again in full.
  • Once you commence a course of treatment and you fail to attend your appointment and / or any other subsequent appointments you may not be offered any further appointments at the practice.

Claiming Free Dental Treatment

If you think you might be enititled to free NHS dental treatment because you or your partner are receiving a benefit, please check your entitlement first. Making an incorrect claim could result in your paying a penalty charge of upto £100, in addition to the cost of your NHS dental treatment.

**It is the patients responsibilty NOT the practices to ensure they are selecting the correct exemption that applies to them when they are signing their FP17PR form (blue consent form) **
