Oakland Community Care Centre is a registered provider of living accommodation for people who require residential or personal care. Everything we do is driven by the needs, abilities and the aspirations of our clients, as far as is practicable. We are committed to the principle of consultation and full involvement of clients (or their representatives) at all stages of their care.
Oakland is available to clients with residential care needs and specialises in dementia – we have a 32 bed residential unit which accommodates 20 long term residential care beds and has 4 beds for short term or respite care, specialising in dementia. In addition, we have one 8- bed wing designated for Intermediate Care – these clients would have the benefit of a Physiotherapist and an Occupational Therapist on site in addition to the assessment and planning skills of the residential management team to facilitate rehabilitative care during an illness or after hospital care, before being discharged home in a supportive way.
At this facility, there are also 88 apartments which offer Extra Care – rented or owned accommodation into which a care package may be arranged, in consultation with the occupiers. In addition, there is a dedicated Day Centre which offers support to clients from the South Derbyshire community across a range of specialist needs. The property is set in Swadlincote, which offers a convenient town centre location, at the heart of the community of South Derbyshire.
We provide a range of health and social care support in partnership with other agencies. We provide a regular programme of social activities, hobbies and leisure interests. This includes the choice and opportunity for people to participate as fully as possible in local community activities.
We support people’s rights in care, including their continued links to participating as a citizen of the wider community, and to have emotional and social needs accepted and respected.
