   It should be standard procedure when patients name and date of birth of Patients are asked by the Surgery receptionist in the Surgery, this should be requested discreetly without other people listening in accordance with GDPR. Also, I personally requested for my consultations to be at Fieldway Surgery about 2 yrs ago with the surgery manager/administrator due to disability. I am baffled that I still get referred to Headley or Parkway as receptionist keeps stating that that my request is not on record. I will like both issues to be looked into accordingly Regards N/A
