    My father passed away recently after a short illness, he was discharged from the local hospital to die at home. District nurses from the practice were seeing him alongside the care team from the hospice. We have no complaints about their service who were fantastic and supported my mum. After his death we are struggling through our grief to work out why the communication has failed and why we are still waiting for a doctor to complete the paperwork, so that we as a family can start the process of life without him. Until today we believed everything was proceeding for the arranged funeral date, with his son travelling from New Zealand to attend. Worried that the registrar had not contacted my mum yet, lead to the discovery that the death has not been certified a week after his passing. This is a group practice and the doctor who last saw my Dad 30 days ago is currently on leave, why couldn't another doctor certify as the last visit was outside the 28 days anyway? This is adding to the distress of the situation and the practice had not notified anyone, my mum, the funeral director, or the registrar of the delay. My Dad was sent home to die peacefully, so the outcome would not be a shock to any medical professional. Ironically my Dad's career was working for the NHS as a Community Dialysis Nurse and in this emotional time the family need to lay my Dad to rest without the extreme distress this is putting on us all in particular my mum. We are unsure if the paperwork will be completed in time for the arranged funeral and my brother has booked his flights to travel from New Zealand. The card my Mum received today from the surgery although intended to give comfort has done nothing more than upset and distress her further addressed to the family of the late (name of my Dad), my Mum is also a patient at the practice and I'm sure she will be listed as the the next of kin, a more personal touch would be to address it to her! Inside there are offers of support and help from the practice, the only help we need from yourselves is to get the paperwork done so that we can organise our farewell to a great man.

  2. Review titled totally uncaring for the frail elderly living alone

    Since last year I have written and asked that someone make a home visit to a distant relative who lives alone and has not been out of the house for 4 years. She is 94 ,I live 200miles away-she has no children. She has lived in the same house for 60 years and has attended the same practice, but been healthy. She has had dementia since pre-covid. Only thanks to a neighbour has she had 1 covid jab!! The elderly neighbour was also doing shopping for her. It has come to the point that food he buys is going mouldy in the fridge and she requests items she already has! I cannot even have a phone chat now as she asks the same question repeatedly(within 5 minutes) and she seems v deaf (? glue ear).I visited last week-she can hardly walk as her feet are so sore. She goes upstairs to toilet as the downstairs one is broken.(denies it needs mending as has one upstairs) She could not make me a cup of tea without guidance. She is reluctant to let anyone in she does not know and remains v independent . A social services worker visited last Sept. (stayed on the doorstep) for 'a chat'-her conclusion to me was" she's coping well, but took a while to get to the door".. If only a local psychiatric social worker could visit , they would be able to identify her mental state . I have asked that the Dr check her covid status, get a podiatrist to visit, a hearing check and mental state assessed. Because of the dementia she does not think anything needs doing so declines "help"apart from her kind neighbour. I have never had a reply from this practice but have just heard she night be having a chiropodist to call-please can I be consulted re this so that I can be there with this lady? To date Social services can't help as she refuses help and last year a Dr suggested she 'attends' a memory clinic! "Why would I do that?" she says" Nothing wrong with me" I am writing this as it seems impossible to get thro' (via phone)to the practice so my requests etc to date over some 3 years have been via Royal Mail-with no reply. Help your patient, before she is found dead at the bottom of the stairs!! PS My mother and my sister had Alzheimer's disease- so I have had experience of this mental state.

Unfortunately, we are unable to identify this lady from the information given here. We would very much like to help and support this lady and ensure that any additional care can be facilitated and put in place to meet her future needs.
