    My father recently admitted following discharge from hospital, as we live away we were contacted by the hospital to say where he was going and that when he got there we would get a call, we had no call from them and on trying the phone was ringing out not even an answer machine to leave a message. Dad had a fall within a couple of day of being in there and no one answered his fall alarm he ended up ringing us for help! We live over an hour away! We managed to get hold of the centre eventually and asked how he was to be told ‘yes he has had a good day’ we then said he was currently on the floor waiting for help and had been there for 40 mins! We were told there were only 2 staff working on that floor! I appreciate there are staffing issues and shortages but 2 staff for about 20 patients is dangerous. He has had very little in the way of physio since he has been in and we have had very little in the way of communication. Another incident was a general welfare call where we asked for an update as he was waiting for a routine hospital visit. The hub said there was nothing through and nothing in the handover. We got a call the next day from the hospital asking how soon we could get there as he was there for his routine appointment! He had been sent via patient transport so obviously someone knew about it! We got to the hospital and went through dads medical history, we were sorted in less than an hour of being there dad spent 12 hours in a hospital waiting mostly for patient transport. This could have been avoided had we known about his appointment as we could have driven him to and from the hospital. He was sent without a chaperone with a serious brain condition. I know patient transport has to be arranged in advance so how come no one knew about his appointment? The communication in the hub needs serious attention. If we had known and picked him up for his appointment we would not only have been helping the overstretched transport service but also the hospital by getting a vulnerable person in and out of an already stretched a&e. Dad is only 1 of many people in the hub but we know of at least 1 other that asked for help on 1 of our visits as she was desperate for the toilet we assisted this lady to the bathroom because no one answered! Uti’s are serious especially in older people so this could have resulted in something more than an embarrassment to this lady.
