How to make an appointment

Please call our main switchboard number on 0113 295 4770 to book an appointment. Urgent cases are seen on the day. If your condition is non-urgent, you can expect to see a GP within two working days, though you may have to wait longer if you want to see a particular GP. If you don’t need an appointment within two working days, you also have the option to book up to four weeks in advance if this is more convenient for you. Nurses based in our practice treat patients for a wide range of common conditions. You can expect to see a nurse within one working day. You can book up to a month ahead for clinics – please see the ‘services and clinics’ section.

Christmas 2016 opening times

With effect from Saturday 3.12.16 we will be offering appointments from the Armley Hub, located at Priory View Medical Centre, 2a Green Lane, Leeds LS12 1HU. The service is open every Saturday from 8am to 2pm, and some periods over Christmas and New Year bank holidays.  If you would like to make an appointment please dial 0113 295 4260 (only during Hub opening hours). You may not be seen by your usual GP.  If your call falls outside normal surgery and Hub hours and you feel you need to be seen urgently please contact NHS 111.

How to register

The Practice welcomes new patients. If you live within the Practice boundary described in our Practice Leaflet you may register with the Practice. The Practice boundary includes Wortley, Farnley, and certain parts of Armley, Bramley, Pudsey and Gildersome.  Please check with our reception staff if you are unsure whether you are included in the Practice boundary. The reception staff will supply the necessary forms for you to complete, and you will be asked to attend for a New Patient Check with one of the Practice Nurses. Either call in at Hawthorn Surgery, on the second floor of Wortley Beck Health Centre, or ring 0113 295 4770, to obtain the registration forms.

Our approach to your healthcare


We welcome all patients to the surgery. Our aim is to provide up to date family medicine without losing the old fasioned qualities of personal care. In June 2008 we moved premises from Branch Road, to the second floor of Wortley Beck Health Centre, which is a state of the art health centre and from which we can expand our services to patients. We have recently started a Physiotherapy clinic, and plan to introduce a Weight Management clinic and Primary Care Alcohol Service. Our patients are of paramount importance to us, and we take pride in offering the best quality of care possible. The Practice staff are highly trained, friendly, and approachable.

How to get your test results

On average please allow 5 working days for your test results to come back from the hospital unless your doctor has advised otherwise. Please note that X-ray results take a little longer, usually 7 days. To check whether your results are back, please telephone the surgery on 0113 295 4770. Sometimes, we cannot always give full results over the phone. If the result is complicated, or if the doctor wants to see you about the result, we will offer you an appointment. We will only give results to the person who has had the test.

Instructions on what to do with the registration form

After you have returned your registration forms to Hawthorn Surgery, and been accepted, your medical records will be transferred to us and a new medical card sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

How to get a prescription

Some patients on regular medication can get a prescription without seeing the doctor each time. The repeat prescription system is computerised and each repeat prescription has a tear-off slip called the Repeat Prescription List (RPL) which shows your regular medication. Please keep the RPL. You or someone on your behalf just needs to post in into the Prescription Box at the surgery when you need more medication. After 2 working days a new prescription attached to an up-to-date RPL is printed for collection. If you cannot come to the surgery in person, just send the RPL with a stamped addressed envelope. We will only be able to send prescriptions to the address you are registered at, to minimise prescriptions going astray. Requests for repeat prescriptions are not taken by phone for safety reasons.
