Summary Care Records and Healthspace Accounts

Summary Care Records (SCR) will be going live in our practice on Tuesday 23rd November

This is a service that will ensure NHS staff treating you will have information about you that might change the way you are treated.

The information that will be included will be:

• Contact details
• Allergies
• Current and past medication

You should all have recieived information regarding this through the post, if you didn't want a SCR, then you should have filled an opt out form, but if you have changed your mind recently then please contact the surgery and they will sort it out for you.

Healthspace

This is a service that is linked to SCR. It is a website that can be accessed online which enables you to see a summary of the information that the NHS holds about you, This means that you can;

1. Check accurate and up-to-date information
2. Check Medication
3. Track other health information such as hospital appointments.

To access this, please visit www.healthspace.nhs.uk , you need to be over the age of 16yrs old and live in England
