Overview

NEW PROVIDER ANNOUNCED  

As you may already be aware, GP services at Seaforth Village Surgery are currently provided by an Interim Provider. This contract to deliver services at Seaforth Village Surgery expires on 31st March 2017.

NHS England (Cheshire and Merseyside) have been working to make sure that a permanent provider is put in place so that you can continue to access high quality services at your GP practice.

NHS England have now confirmed a new provider for Seaforth Village Surgery; from 1st April 2017, services at Seaforth Village Surgery will be provided by Urgent Care 24 (UC24).

Under this contract, Urgent Care 24 will also provide primary care services to Litherland Practice Seaforth Village Surgery will continue at the existing premises at 20 Seaforth Road, Seaforth. However, UC24 intend to run Seaforth Village Surgery and Litherland Practice as one practice under a new name and they will contact all patients about this in the near future.

UC24 will be:

  • Writing to all patients to introduce themselves, and to arrange ‘meet and greet’ sessions where patients can ask any questions and raise any concerns
  • Engage fully with the local Patient Participation Group
  • Display posters in the practice informing patients of the change in provider and who patients can go to if they have questions

When our Practice is closed, please ring the surgery number 0151-949-1717 and listen to the instructions on the answer machine. This will inform you of what to do next.

If you have a life threatening medical emergency then please do not contact the surgery please call 999.

In the event of minor ailments please contact the Walk-in-Centre 0151 4754667.

If your reason for calling the surgeyr is of a non urgent nature then please wait until the surgery re-opens.

Welcome To Seaforth Village Surgery.
There are many ways to contact us Here at the surgery, in person, via phone, writing or online at patient access. If you would like further information on patient access or any other services, please contact one of our reception staff who will be happy to assist you further.

We have a HCA on board with us now at the surgery.We offer phlebotomy services, blood pressure reviews, weight management, new patient medicals and nhs health checks. Appointments can be booked in advance.

We have 2 Practice Nurses, who offer chronic disease management, cervical screening, childhood immunisations, Travel consultations and vaccinations, Seasonal vaccination programmes and a friendly ear in times of need.

As the Practice manager i am always contactable during surgery opening hours, you can contact the surgery and leave a message (the staff will let me know) or email me on the following address.
