Cornwall Partnership NHS Foundation Trust provides a range of community mental health services to support people over the age of 18 with a severe mental health illness.
Community Mental Health Teams (CMHTs) are where most people receive support for their mental health problem. They help people to manage and recover from a range of common mental health problems.
CMHTs provide one-to-one, individualised support that may take the form of visits to a person’s home or at a community setting such as a GP surgery. They also work closely with our other mental health teams and services, such as:
