Who is it suitable for?
Changing Health is for people aged over 18 who have type 2 diabetes. It is not suitable for people on insulin.
How does it work?
To access a Changing Health programme through the app, you will need a username and a password. This will be sent to you by email following a referral from your GP.
You’ll work through a short educational course using the app, which explains how your diabetes affects you. The app is full of resources to help you lose weight, eat better and move more – all designed to help improve your condition.
After that, you’ll be put in contact with your own dedicated lifestyle coach who will create a personalised diet and exercise plan based on your own needs and circumstances. They’ll monitor your progress and follow up regularly to help you stay on track, giving you the best possible chances of success.

How do I access it?
Changing Health is available in a number of areas across the United Kingdom, with more locations being added all the time. Ask your GP whether it’s currently available from your surgery, and if so, they’ll refer you onto the programme.
You’ll receive instructions by SMS and email to get you started, and you can then download the app for free from the App Store or Google Play.
