Community nursing

This service is managed by Kent Community Health NHS Foundation Trust

We are a skilled team of professionals who include community nurses, district nurses, community matrons, specialist nurses, health care assistants and therapy assistants. We provide all your nursing care in the community.

Our community nursing teams ensure patients can be cared for in their own homes, including residential homes, where appropriate. We work hard to treat you effectively at home so you do not have to go into hospital unnecessarily. We look after people who are housebound and who have nursing needs. A range of nursing procedures are carried out, for example, medicine administration, taking blood, dressing changes, assessments and end of life care. 

We work closely with other agencies, such as out-of-hours and social care services and in many places are operational 24-hours a day. In some parts of Kent, we operate a rapid response service, which aims to assess patients quickly and prevent unnecessary hospital admissions.
