NHS Calderdale Clinical Commissioning Group is the CCG covering 26 GP practices and a registered population of more than 213,000 patients. CCGs are groups of GPs that are responsible for planning and designing local health services in England. They do this by ‘commissioning’ or buying health and care services including:

• Planned hospital care
• Urgent and emergency care
• Rehabilitation care
• Community health services
• Mental health and learning disability services 

Clinical Commissioning Groups work with patients and health and social care partners (e.g. local hospitals, local authorities, local community groups etc) to ensure services meet local needs. CCG boards are made up of GPs from the local area and at least one registered nurse and one secondary care specialist doctor.

The CCG is made up of local clinicians who are working together to secure the best possible healthcare for local communities. Our aim is to improve the health and lives of local people by increasing life expectancy, making sure we commission and provide good quality services and to reduce health inequalities across the district.

Our priorities

As an organisation we are working towards six key priorities. These are:

1. Preventing people from dying prematurely
2. Enhancing the quality of life for people with a long-term condition (inc work on urgent care pathways)
3. Helping people to recover and maintain their independence (inc work on intermediate tier)
4. Ensuring people have a positive experience of care (inc those in care homes, and those accessing primary care)
5. Ensuring a safe environment and protecting people from harm
6. Reducing inequalities in Calderdale.

Care.data awareness campaign rolls-out in the north of England
