  • A Summary Care Record will be created for each patient. 
  • This means if you visit an emergency service anywhere in England, the person responsible for treating you, will be able to, with your permission view which medication you are taking – this means the person who is treating you will be able to offer you the best care possible, at the point of care

 GP 2 GP Patient  Records Transfer

 

The Practice has introduces this  facility to transfer patient records between practices from June  2014, this facility will be used when a patient registers or leaves the Practice.

 

This new process will reduce delays in your medical records being received by Everton Road Surgery  or being sent to your new Practice and will allow your records to be available to the Doctors and teams as soon as you require an appointment.
