Although I have been asked to provide a date of my last visit this is actually the date I last tried to make an appointment. Contacted the surgery as I have an interesting lesion developing on one finger. I was asked to provide a photo ahead of a telephone appointment. This I was willing to do, but was unable to take a photo with sufficient detail to be of use so let the surgery know. Despite the fact that the doctor would be unable to see the lesion the practice still wanted to proceed with a telephone appointment first. Why they cannot see that in such circumstances this is a total waste of not just my time but also the doctor's time is beyond belief. Telephone appointments may have some utility in some circumstances, but there are times when they are utterly pointless.

This the worst GP I ever had. It get even worse after COVID. All the appointments through the phone. You can not schedule an appointment with doctor next day. You have to call early morning so the doctor will call whenever suitable for them without time arrangements. They never call to follow up any test or x-ray results.
