From lst June 2010 the practice offered Summary Care Records service (SCRs) as part of the NHS Care Records Service to improve the safety and quality of patient care.  This means that your records will be available to people providing you with care anywhere in England.  At first, details such as allergies, current medication and any bad reactions to medicines will be detailed.  You have the right to refuse this service, if you wish.  Please contact Reception for further details.   If you do not wish your records to be shared, you will need to notify the Practice and sign the dissent form.

PNEUMONIA VACCINATIONS

Pneumonia Immunisations for Over 65's and those patients with chronic disease

e.g. Asplenia, chronic respiratory disease, chronic heart disease, chronic renal disease, chronic liver disease, diabetes, immunosuppression

Pneumonia vaccinations are available to the above patient group. Please make an appointment with the Practice Nurse by contacting Reception

This vaccine is a single dose providing life time cover.

Please contact Reception for clinic details.  If you are unable to attend the designated clinic then we will be happy to make an appointment for you to have the immunisation at a time to suit you.
