Registering as a Patient ~ Our Receptionists will be happy to assist you and it would help if you would give us your medical card and proof of identity and residence (ie a passport). You will be offered a new patient check with one of our new practice nurses to enable us to take note of your current health status. It is very important for you to attend. Please make every attempt to come for this check up. If you cannot attend, however, you should let us know and arrange to make another appointment as soon as possible. Registering with the Practice ~ Registration forms and a pre-registration questionnaire need to be completed and handed into reception. Please pick up a form at reception to complete. Alternatively, the pre-registration questionnaire form can be accessed by clicking on the 'More Patient Information' link below. Kindly print off the form, fill it in and bring it to the Surgery when you come into register. ~ All new patients must have a new patient check within the first six weeks of being accepted. Potential registrants should check that they live within the practice boundaries.
