TRAVEL VACCINATIONS 

Lindley Village Surgery

If you are planning to travel and need some

 

advice please collect a Travel Form from

 

Reception or visit our website at www.lindleyvillagesurgery.nhs.uk where you can download a form which you will need to complete and return to the surgery.  

 One of our practice nurses will then use the

information you have provided to determine

whether or not  vaccinations are

recommended. 

  You will need to allow 14 days

 between returning your completed form and

contacting us for advice.  Please ensure you

return the form to us at least 10 weeks prior to

 your departure date in order to allow sufficient

time to complete any vaccinations which may

 

be required.

 

 

Please note that we endeavour to give you the best

 

most up to date advice possible for your travel

 

health needs.  We are not, however, a travel

 

advice service and therefore cannot be held

 

responsible for inadequate information given .
