We are currently accepting new patients

Please come and register every Tuesday and Wednesday between 3pm & 5pm! How to Register withClayhall Clinic. To register with the practice as an NHS patient, please present your signed medical card to the receptionist. If this is unavailable, we have spare registration forms in the Surgery for you to complete. Each patient over the age of 5 years will require a free and mandatory new patient registration health screen. Please bring a fresh urine sample with you for this examination. Children under the age of 5 will be entered onto our Child Health Surveillance Register which works in tandem with the Child HealthClinic locally.
