Important Information about your Health Records

Eagle house Surgery takes part in the NHS Care Records Service. The NHS will create a Summary Care Record for you, which can be used by NHS clinicians treating you, in emeregency and out of hours situations. Staff will only be permitted to access information if they are involved in your treatment, with access to your record being controlled by strict security measures. If a healthcare professional needs to view your Summary Care Record, your permission will be asked beforehand, unless you are unable to communicate your wishes at the point of care.

Details of allergies, recent prescriptions and previous bad reactions to medicines, along with important information on any problems or conditions you may have, such as:

  • Significant illnesses and health problems from the past, such as a heart attack.
  • Illnesses that you take regulare medication for, such as asthma or diabetes.

Further types of information that may be added in the furture are:

  • Important test results and immunisations
  • Information you might have given your GP about how you want to be treated in an emergency, such as not wanting a blood transfusion

If there is any information that you do NOT want to be included in your Summary Care Record, you must let your GP know.

Should you choose not to have a Summary Care Record you can:

  • Collect a form from your GP practice, complete and return it to the practice.
  • Attend one of the Information Sessions, collect and complete the form.
  • Download a copy of the form from our website and return it to your GP practice.

 Please note each person must sign their own request form.

Swine Flu

If you are suffering from the possible effects of Swine Flu: Feverish illness with high temperature, Cough, Runny nose, Sore Throat, aches and chills, or diarrhoea and vomiting, please telephone the Surgery for advice before you make an appointment.   Please do not come to the surgery without requesting advice first.

Swine Flu Immunisations

The  group of patients that the Department of Health advise should be immunised are those aged 6 months to 64 years inclusive, who fall into the following catergories:

Patients with Chronic lung disease, Chronic heart disease, Chronic kidney disease,  Chronic liver disease, Chronic neurological diseases, Diabetes mellitus, or patients who are Immunosuppressed caused by treatment or disease.

Pregnant women at any stage of pregnancy and

People who live on the same house as someone whose immune system is compromised by disease or treatment.

The vaccine is also being offered to health and social care staff who may be in close contact with people in the above groups.

Please do not make an appointment yet if you do not fall into any of the catergories above. However, if you think you or your child should have received an invitation please contact the
Surgery.
